Teacher Verification Checklist

☐ Teacher’s full name
☐ School name
☐ School district name
☐ School website link or school email address 
☐ Grade level(s) taught or subject area
☐ A photo or scan of one of the following:
Current teacher ID badge (with sensitive numbers covered if applicable),
Staff directory listing showing their name and school
☐ Shipping address or shipping address of the school (If preferred over home address)
☐ Brief description of classroom needs (optional, for matching supplies)
